The biology, briefly

Cholesterol can't float on its own, so it rides through your blood inside protein-wrapped shuttles called lipoproteins. HDL is the shuttle that carries cholesterol back to your liver for cleanup. The rest, LDL and VLDL and the triglyceride-rich leftovers, ferry cholesterol outward, where some of it can lodge in artery walls.

Non-HDL simply adds up all that outward-bound cargo. Your lab finds it by subtracting HDL from your total cholesterol, so no fasting is needed, and it picks up risk that an LDL number alone can miss.

What your number is telling you

Non-HDL reads out the cholesterol most likely to build plaque in your arteries, the slow process behind heart attacks and strokes. Because it counts particles that LDL testing can overlook, it often tracks heart risk just as well, and sometimes better, especially alongside diabetes or high triglycerides. Lower is better here.

A higher number is a useful early flag, and the upside is that it's one of the more movable numbers you have. Diet, movement, weight, and medication can each bring it down meaningfully. Read it as one clue among several, not a diagnosis, and let your clinician weigh it next to your other markers and your personal risk.
